Complete the online UTVS Fall 2023 Staff Contract Form .
The UTVS Membership fee is $20.00. UTVS Members must submit their membership fees using the membership fee dropbox located in the TV Studio Lobby.
The TV Studio Lobby is open Monday through Thursday from 9:00 am through 5:00 pm. Closed Friday, Saturday and Sunday.
UTVS Members must sign their name, print their name, print the date, and place the form and their membership fee of $20.00 into the provided empty envelopes, and then drop the sealed envelope with their money and completed form into the membership fee dropbox.
Both the online UTVS Fall 2023 Staff Contract Form and the membership fee forms must be completed.
